Nodular invasive pulmonary aspergillosis post allogeneic BMT, treated with oral itraconazole

Case number:

Patient LC. A 43 year-old post allogeneic BMT recipient developed nodules in the lungs 12 weeks after a successful transplant. A needle biopsy grew A. fumigatus. Pre-existing renal dysfunction was problematic so she was treated with oral itraconazole capsules for 3 months. Her nodules regressed in size despite severe GVHD and hepatitis B infection. She died of GVHD. Autopsy showed persistence of Aspergillus in the nodules, without vascular invasion.
