Mucositis Does Not Lead to False Positivity of Galactomannan ELISA Test

Abstract: 

Background: GM monitoring using ELISA method is a routine screening test for early detection of invasive aspergillosis (IA) in hematooncological patients. Major causes for false positivity of the assay were described in recent years (piperacillin/tazobactam, Plasmalyte solution). However on the basis of limited data from the period when these causes where not known, mucositis and passage of GM of food origin from intestine through damaged mucosa into the serum is still thought to be important reason for false positivity of the test. Methods: 2 blood samples in consecutive days and 1 stool sample have been obtained from hematooncological pts. without proven or probable IA (EORTC/MSG) and with different grade of mucositis (WHO grading). Pt. could be included in the study repeatedly if grade of mucositis changed. Blood and stool have been tested for GM using Platelia Aspergillus kit (BioRad, France). Results: 214 blood and 107 stool samples from 77 pts in 107 occasions with different grades of mucositis have been tested for GM presence. Results are shown in the table. GM level in the stool did not correlate with grade of mucositis, % of oral intake or % of pts. with total parenteral nutrition. GM level in the serum did not correlate with severity of mucositis. False positive result of GM test in the serum (isolated not confirmed in the second sample) was identified only in one pt. without mucositis. Conclusions: GM is present in the stool irrespectively of % of oral intake or presence of total parenteral nutrition. Passage of GM through damaged mucosa is very rare, if any. We did not identify pts with mucositis and false positive result of GM in the serum. Mucositis does not lead to false positive result of GM ELISA test.
