Unusual and rare fungal pathogens – A report

Abstract:

Modem health care facilities have improved the living conditions of immunosuppressed persons, but this has resulted in newer
invasive fungal diseases. Apophysomyces is a thermotolerant fungus often causing fatal infection. Ochroconis, an ubiquitous
black mold, is being increasingly isolated from organ transplant recipients. Stephanoascus is a well known opportunistic
pathogen but fatality is less common. Intuition to rule out opportunistic fungi in suspected mycotic illness helps in saving the
infected person. Three cases of unusual fungal infections were diagnosed during one year period. A case of fatal necrotizing
anterior abdominal wall fasciitis due to Apophysomyces variabilis, a case of subcutaneous ulcerative chromoblastomycosis
caused by Ochroconis constricta and a case of oral thrush caused by Stephanoascus ciferrii were reported. Necrosed tissue bit
from a case of anterior abdominal wall fasciitis, tissue bit/discharge from the ulcerative lesion on leg and oral swab from a case
of white patch in oral cavity were examined employing different mycological diagnostic methods. Macroscopic and
microscopic evidences helped to identify three rare fungal pathogens. Gene Sequencing and phylogenetic analysis identified
Apophysomyces variabilis. Phylogenetic analysis identified O. constrica and S. ciferrii respectively. Knowledge of newer
pathogens and proper identification of the unusual pathogens helps initiation of suitable antifiuigal treatment. Active
diagnostic bench work and a wide angled thinking of possible fungal disease is very vital.
