Abstract:
Background: Recently the echinocandins are widely used for invasive camdidasis. It was showed the correlation between increased caspofungin usage and an incidence of Candida parapsilosis candidemia. ( Forrest et al. Journal of Infection 2008;56:126-129.) It is still unknown that correlation between micafungin(MCFG) usage and incidende of candidemia. Methods: All episodes of nosocomial candidemia were evaluated from January 2003 to December 2011 in Tokyo Women’s Medical University Hospital (1423-bedded). Candidemia was defined by culture positive for Candida species from blood culture drawn 72 h after admission. An incidence of candidemia per 1000 patient day was evaluated. Defined Daily Dose (DDD) of MCFG, fluconazole(FLCZ) and liposomal amphotericin-B(L-AMB) were collected according to World Health Organization guidelines (http://www.whocc.no/atc_ddd_index/) in this period. The correlations between incidence of candidemia and DDD of antifungal agents per 1000 patient day were evaluated using Spearman`s rank correlation coefficient. Results: All the 274 episodes of candidemia had the following distribution: Candida albicans (41.9%), C.parapsilosis ( 27.2%), C.glabrata(8.3%) in our institute. Incidence of candidemia per 1000 patient day ranged from 0.05 to 0.1 by overall, from 0.021 to 0.042 by C.albicans, from 0.0095 to 0.025 by C.parapsilosis. DDD of antifungal agents per 1000 patient day were ranged from 1.17 to 6.91 in MCFG, from 1.01 to 3.71 in FLCZ, from 6.58 to 12.6 in L-AMB. No correlation was found between DDD of MCFG and C.parapsilosis (Rs=0.577, p=0.163). Conclusions: To our knowledge, this is a first english report about correlation between DDD of antifungal agents and incidence of candidemia in Japan. Micafungin usage didn’t affect an incidence of Candida parapsilosis candidemia whereas caspofungin usage.
