Abstract:

Standardized methods for antifungal susceptibility testing have been published in excess of ten years. Despite this fact, much confusion surrounds interpretation of these results. Adding to the confusion is the fluid process by which CLSI standards are developed. This fluidity permits changes as our understanding and knowledge base grows.
Prior to the current standard, method changes were quite minor and encompassed primarily the addition of new compound QC ranges or breakpoints. Approved but still pending publication, the newest version contains extensive changes. Primarily, breakpoints for Candida spp. will no longer be valid and guidelines for breakpoints will be species and drug dependent. In addition, a new concept for interpretation of antifungal testing is being introduced. Epidemiological Cutoff Values or the ECV will be a new addition to the documents. The ECV is determined for species where sufficient clinical data is unavailable for purposes of defining a breakpoint. By comparing the ECV to MICs obtained through testing, it is possible to identify isolates that may possess resistance mechanisms. Of great interest is that ECVs will be provided for both yeast and mould species.
