Chronic invasive sinus aspergillosis caused by Aspergillus terreus

This 49 year old diabetic man presented with epistaxis and nasal obstruction. Localised swelling of his face was apparent. The mass in the maxillary antrum was removed by maxillectomy and yielded Aspergillus terreus . He received no antifungal therapy. 6 months later he presented and localized extension was seen on CT scan. The mass was partially removed surgically, and he received amphotericin B and then itraconazole. Unfortunately he relapsed with localized extension to his brain and died shortly thereafter.
